A CASE OF CARBON MONOXIDE INTOXICATION ASSOCIATED WITH ACUTE RENAL FAILURE



Abstract
A case of acute renal failure after CO intoxication associated with ischemic muscle injury of the lower extremities is presented.
A 19 year old Korean male manual worker was found in coma due to CO intoxication and resuscitated by hyperbaric oxygen therpy within 4 hours at another hospital. Because of hematuria and anuria, he was transferred to the Catholic Medical Center 4 days later. The oliguric pahase was characterized by severe hyperkalemia despite 3 peritoneal dialyses in 8 days, which was only relieved by hemodialysis.
The diuretic phase, which started on the 13th day, was complicated with urinary tract infection due to klebsiella.
The patient was discharged on the 19th day with good renal function(creatinine clearance 65 ml/min) but with residual ischemic myopathy and neuropathy of the lower extremities. The ischemic myopathy of the lower extreapparently contributed to developement of acute renal failure and pronounced hyperkalemia.
The pathogenesis of acute renal failure in this case is uncertain, but we considered two factors; one direct effect by generalized hypoxia, the other "crush syndrome" by ischemic muscle injury.
